Retinol vs Retinal: What Over-the-Counter Vitamin A Can (and Can't) Do

The short answer

Retinol can modestly improve fine lines, but it is weaker and slower than prescription tretinoin [2], and a systematic review of over-the-counter vitamin A products found the trustworthy evidence thin [3]. Retinal (retinaldehyde) beat retinol in one small trial [5]. Both are worth trying for early fine lines; neither is 'as good as a prescription'.

Vitamin A comes in a ladder of forms that skin converts towards retinoic acid, the active form: Retinyl Palmitate / Acetate / Propionate (weakest) → Retinol → Retinal (one step from active). Hydroxypinacolone Retinoate is a related ester. The further down that list, the stronger — and usually the more irritating.

What the research shows

Retinol works, modestly. In a 24-week double-blind trial with 36 elderly people, 0.4% retinol on one arm reduced fine wrinkling far more than the base cream on the other arm [1]. That was on arms, in people averaging 87, so it does not translate directly to faces. Retinol produced the same kinds of skin change as prescription retinoic acid, but smaller [2].

But the evidence is weaker than the marketing. A systematic review of nine double-blind, vehicle-controlled trials of over-the-counter vitamin A found four showed no difference from the base cream, and the five positive ones had serious flaws and showed only a mild effect on fine lines [3].

Retinal. In a split-face trial with 23 people, encapsulated retinal beat retinol at the same strength on most wrinkle measures, with no side effects [5]. In another trial with 40 Korean women, 0.1% and 0.05% retinal both improved texture and hydration — but with no placebo, that cannot be separated from simply using a moisturiser [4].

Rules, irritation and pregnancy

EU limits. The EU's scientific safety committee set a safe level of 0.3% retinol equivalent for face products and 0.05% for body lotions [6]; the EU has since written limits into law. Whether Great Britain has adopted the same limits, we could not confirm.

Irritation. Start two or three nights a week and build up; peeling and stinging are common. Use sunscreen during the day.

Pregnancy. A review of 654 women exposed to topical retinoids early in pregnancy found no significant rise in birth defects, but the authors said that is reassurance after accidental use, not a reason to use them [7].
