The short answer
Topical tranexamic acid (TXA) at 3–5% performed about as well as hydroquinone for melasma in small trials, with fewer side effects [2][3]. But the only trial that compared a TXA gel with the same gel without TXA found no advantage for TXA [1]. Promising and well tolerated — not 'clinically proven' in creams.
What it is
TXA is a synthetic version of the amino acid lysine, long used in medicine to reduce bleeding. On labels look for Tranexamic Acid. Results from TXA taken as tablets are often quoted in marketing; they do not transfer to creams.
What the research shows
Versus a plain base. In a 12-week split-face trial with 23 people with melasma, both sides lightened — the side with 5% TXA gel was not significantly better than the side with the gel alone, and TXA caused more redness [1].
Versus hydroquinone. A 50-person split-face trial found 3% TXA improved melasma as much as a hydroquinone-plus-steroid cream, with fewer side effects [2]. A 60-person trial found 5% TXA matched 2% hydroquinone, and more people were satisfied with TXA [3]. Neither used a placebo.
Pooled results. A meta-analysis of 11 studies (667 people) found TXA reduced melasma severity with minor side effects, but it mixed tablets, injections and creams together [4].
Who it suits, and cautions
TXA is a reasonable, gentle option for uneven tone and post-spot marks, especially if you cannot tolerate hydroquinone or strong acids. Some people get redness [1]. Most Korean TXA serums and toners list it at lower levels than the 3–5% used in trials, so expect gradual, modest change and pair it with daily sunscreen, which matters more for pigmentation than any serum.
